Dr. Shammona Clark

DNP

Has devoted her career to providing exceptional healthcare in medicine and has been passionate about the subject ever since a young age. NP Clark leads our team of experienced and dedicated providers and clinicians. She runs a wide range of clinical simulations for students interested in providing individualized and comprehensive care.

With over 20 years of experience including Regional Nurse Consultant, Director of Nursing, Reimbursement Specialist, Charge nurse, Wound care specialist, Psychotropic management nurse, Infection prevention supervisor, Quality assurance nurse, hospice care, LTC Administrator, and Director of clinical information systems. NP Clark has devoted her career to healing and helping those entrusted with her care.

As a provider, her duty and goal are to give you and your family the care you need and deserve. She takes pride in supplying my clients with extraordinary and individualized care. I enjoy what I do, therefore I pay close attention to the needs and requirements of all my clients and patients. I look forward to assisting you with your plan of care. She earned her licenses practical nurse certificate in (2007) and her Associate's Degree in 2012 from Ivy Tech Community College. She then earned her Bachelor of Science in Nursing from Purdue University-Calumet in (2015), and her Master of Science in Nursing in (2019). Additionally, she completed her Doctoral degree in Nursing Practice (DNP) from Chamberlain University in the spring of 2023 and the University of St. Francis Psychiatric Mental Health Nurse Practitioner program in December of the same year.

Dr. Talia Thompson-Jones

As a devoted Registered Nurse, Family Nurse Practitioner, and Doctor of Nursing practice and health care provider for over 20 years, I have applied my skills and expertise to providing exceptional and patient centered healthcare. I’ve been helping my patients and clients find their optimal wellness through both traditional medical services, complementary medicine, and holistic and alternative elective medical treatments.

Affectionately known by my patients as Dr. Tee, I help to develop a deeper understanding of each individual client’s goals for their health, their habits, and their lifestyle choices to reach each client’s optimal level of health.

As a primary care provider, I aim to create a healthy life balance that prioritizes individualized well-being and celebrates all aspects of true self and to start nurturing the body and soul.

Dr. Tee is a board-certified Family Nurse Practitioner. She earned her Bachelor of Science in Nursing (2005) from Chicago State University, Illinois, Master of Science in Nursing Family Nurse Practitioner (2016) and Doctor of Nursing Practice (2022) from Walden University, Minnesota. With 20 plus years of nursing experience, national board certification with the American Nurses Credentialing Center (ANCC) as a family nurse Practitioner, I have worked emergency medicine, primary care, skilled nursing, assisted living, infectious disease, weight loss and nutrition, and medical aesthetics.
